If appropriate, which regiment and or section did you serve with in The British Armed Forces in the 1990/91 Gulf War? (Reservists and dependants are also requested to complete this question)

Have you ever given evidence on any previous inquiry or panel review in connection with Gulf War Illnesses, and if so what was the Inquiry and when did you give your evidence?

For Gulf War Veterans and dependants. In the space below would you please detail in no more than 500 words your experience, knowledge or expertise in connection with the deployment of British Troops during the first Gulf War of 1990/91. Would you please describe as best as possible the following: dates, times, places, medical immunisation information, exposure to contaminants, medical conditions/symptoms before, during and after (both diagnosed and undiagnosed) the first Gulf War, and other relevant information.

Please do not send any personal items (such as medical records, photos etc) to the Inquiry office with your submission of evidence form. These items may be required at a later stage if it is deemed necessary by the inquiry.
